NExpress library card application forms
Thanks to Wellsville City Library for contributing the basic design of these Library Card application forms. They are designed for easy but complete data entry in the Add New Patron screen. There is one form for adults, and one for children. You can customize them with your library’s name, and any additional fields you want. Libraries that collect demographic data should add lines for the various patron categories they use.
These applications are designed to be front-and-back duplicated, with an Internet User Agreement form on the reverse. The form mentions an additional “guidelines on computer use” document. If you don’t have a separate Computer Use document, you may either delete this line, or incorporate your user rules into the User Agreement text.
